St. Benilde CYO Membership Forms (2025-2026)

We are excited you want to join St. Benilde's CYO! Here at St. Benilde, we are striving to create an environment for each teen to nourish their Catholic faith through formation, fellowship, service, and fun. 

Please complete one form per teen in household. For payment, please select the appropriate option corresponding to each teen within a household. Payments can be made via credit card (online below), cash, or check (Payable to St. Benilde). St. Benilde/the Archdiocese of New Orleans requires membership forms, medical consent, and liability forms to be filled out and signed.
